Dear Parish Clerk,
I am contacting you as the Diabetes Prevention Lead at NHS Somerset Clinical Commissioning Group. In Somerset we have a nationally commissioned diabetes prevention service for people who are at high risk of developing type 2 diabetes. Usually identified by their health practitioner they are referred to a 9 month behaviour change programme where they are can access learning on diet, weight management, stress and other factors to reduce their risk.
Due to the pandemic we have seen a fall in recent referrals and due to the subsequent pressure that our GP practices are facing, we have enabled a self-referral option via the Diabetes UK ‘Know Your Risk Score’ – a check list of questions that can determine a person’s risk factors – This has also enabled the programme to become more accessible to people in areas of deprivation and BAME communities.
We would love to put some local advertising of the programme in your parish magazine, website and/or social media sites if you have them to spread the word that this programme is available, completely free of charge through the NHS. If this would be possible I would be really grateful if you are able to provide me with dates of publication and any cost implications.
